Administrative License Revocation (ALR) Hearing Request Form

ALR hearing requests for blood failure cases under Texas Transportation Code Chapter 524 should not be submitted until a notice of suspension is received from DPS.

If the request is submitted within the required 15 days after the notice of suspension is received, DPS will send a letter to the address provided on the hearing request. The letter will provide the date, time and location of the hearing. Requests made after the required 15 days will be denied and the individual will be notified by mail.

NOTE: It may take 30-60 days for a hearing to be scheduled.
